My elderly frail parent was just transferred to short term rehab today. I didn't have a chance to tour facility beforehand, but a friend has a parent in their longterm unit that is pleased with their care and the place has decent Medicare rating. I was horrified when I got there this evening. The drywall around the plug to my parent's bed is cracked and falling into the floor. The bathroom needs repair and has a dented heat register. The food my parent was given for a meal was inedible. No one could tell me who was in charge of his care plan, what the immediate plan was until he was evaluated by physical therapy and the facility medical team. They didn't even know if he still had a catheter or if he'd been changed into a diaper. Only one person was wearing a visible name tag, so I had no idea who was an RN, nursing assistant, etc. He was treated rudely by the person who came to give him a bedpan since he cant get up to use the bathroom. I felt awful leaving him there, but was told by one of the nurses there was no one to talk to until Monday. Can I get him into a different rehab?
So please report your findings. Also look up who is regulating this rehab and report all you are finding. They will be particularly impressed with the lack of care, care plan and the personnel without nametags.
Speak to the doctor of your patient, and demand to see the rehab facility MD. They are in violation of licensing on any number of fronts. I know that isn't your main concern at this time, but take pictures and follow up and try to get some state regulators in if you are not paid attention to.
The Hospital Social Workers involved in this transfer should be fully informed. Take them pictures.

I sprung into action like the Energizer bunny. Found a great, clean, well run rehab SNF and they sent a mini bus to the hellhole to pick poor mom up. I was packing her up when they arrived. I was scared stiff to leave her alone for fear the staff would kill her via negligence. She was lucky to escape with her life. The review I left them was horrible, well deserved and long overdue.
Good luck. Don't stand for this. Get your parent out of there asap and leave them a review to spare others the nightmare you've been through.
